Tubo-ovarian torsion (pediatric)

The right ovary is in the midline, being markedly enlarged, showing peripherally displaced follicles with T2 hyperintense central stroma. It is seen eliciting intermediate-to-high signal area within, mainly along its right lateral aspect being continuous with the adjacent tube representing the twisted vascular pedicle. The right ovary is measuring 6.7 x 5.3 cm compared to 2.1 x 1.3 cm of the left ovary. On postcontrast series, the right ovary shows no significant post contrast enhancement. Associated mild free pelvic fluid is seen.

Laparoscopy was performed and the operative findings were four twists of the right Fallopian tube and necrotic right ovary resulting in right oophorectomy and left ovariopexy (surgical fixation of ovary to the pelvic wall).
