Presentation

Gradual onset of right-sided mild scrotal heaviness

Patient Data

Age: 35 years
Gender: Male
ultrasound

There are multiple small cystic/tubular anechoic structures that replace and enlarge the right testicular mediastinum. No mass.

There was no abnormal vascularity in either testis.

Normal left testis.

Case Discussion

The geographic shape, lack of mass effect, and lack of internal flow of the cystic/tubuar anechoic structures are consistent with tubular ectasia of rete testis.

Cystic ectasia of the rete testis must be differentiated from other benign intratesticular lesions, notably cystic dysplasia and intratesticular varicocele. Furthermore, as it is mostly asymptomatic, it should be distinguished from the rare papillary adenocarcinoma of the rete testis, in which a solid mass in the rete testis is usually evident.
