Spastic epilepsy

There is cortical thining in paramidline in both occipital lobes and right parietal lobe, with underlying subcortical white matter hyperintensity in both T2W and FLAIR sequences, shine through DW/ADC, denoting focal gliosis and atrophy.

Case Discussion

Paramidline focal gyrial thining and atrophy with subcortical white matter gliosis, giving mushroom-like shaped cortex. This is also known as ulegyria, and is suggestive of chronic ischemic insult.
