There is bilateral parieto-occipital cortical thinning and widened paramedian subarachnoid spaces, giving mushroom-shaped gyri. with underlying subcortical hyperintensity on both T2W and FLAIR sequences and shine-through on DWI/ADC, denoting focal gliosis and atrophy.

Also noted is a small posterior cranial fossa cyst posterior to the vermis and continuous with the foramen magnum, which follows CSF on all sequences, without restricted diffusion on DW/ADC, in keeping with an arachnoid cyst.

Case Discussion

Focal shrunken parasagittal gyri, with subcortical white matter gliosis, giving mushroom-like shaped gyri, suggestive of ulegyria.
