Presentation

Right index finger injury while catching a cricket ball - 5 weeks ago.

Patient Data

Age: 30 years
Gender: Male

The distal end of the ulnar collateral ligament of the PIP joint shows thickening, and hypoechogenicity with an avulsed bone flake. The radial collateral ligament of the PIP joint shows a normal echopattern. Collateral ligaments of the DIP joint are normal. Intact flexor and extensor tendons. Small effusion in the PIP joint palmar recess.

Case Discussion

The patient had a right index finger injury while catching a cricket ball about 5 weeks before the ultrasound. After an initial negative radiograph for a fracture, immobilization was done. Due to persistent symptoms, he was referred for an ultrasound with a suspected collateral ligament injury involving the PIP joint.

The case shows bony avulsion of the index finger PIP joint ulnar collateral ligament.

Injury of the ulnar collateral ligament of the first MCP joint is known as Gamekeeper's thumb.
