Presentation

Evaluation of polydactyly.

Patient Data

Age: 7 years
Gender: Female
x-ray

Plain radiograph of wrist joint shows polydactyly with duplication of ulna, duplication of ulnar side of carpal bones, metacarpals and phalanges, absence of radius and absence of thumb. Eight triphalangeal digits are seen. The middle phalanx of medial index finger is hypoplastic.

Wrist joint

The MRI of wrist joint of the same patient shows polydactyly with duplication of ulna, duplication of ulnar side of carpal bones, metacarpals and phalanges, absence of radius and absence of thumb. There is duplication of ulnar artery.

Elbow joint

MRI scan of elbow joint shows that the elbow joint is formed by articulation of one humerus and two ulnae.

Case Discussion

Ulnar dimelia or mirror hand syndrome is a rare congenital anomaly of the upper limb characterized by absence of the radial ray (including thumb), duplication of the ulna and duplication of the ulnar halves of the carpals, metacarpals and phalanges.
