Undifferentiated pleomorphic sarcoma - arm

Diagnosis almost certain

Presentation

Swelling of the left lower lateral arm for 4 months. Gradually increasing in size, and associated with intermittent pain. No history of trauma.

Patient Data

Age: 50 years
Gender: Female

There is a 30 x 50 mm well-defined soft tissue mass lesion in the left lower lateral arm within the brachialis muscle. The lesion is isointense to the muscles on T1, heterogenous signal on T2/PD fat suppression sequences with internal cystic changes and shows heterogenous enhancement after contrast administration.

No significant fat component or calcifications are seen in it. 

There is no invasion of the neurovascular bundle or underlying humerus.

No skip lesion or significant lymphadenopathy is seen.

Case Discussion

The patient went on to have surgery and the histopathology examination was compatible with undifferentiated pleomorphic sarcoma. Immunohistochemically, tumor cells react positively for CD 68 and SMA while negatively for Desmin, MDM2, S100, SOX10, LCA, and CKAE1AE3.
