Presentation

Lytic lesion noted on radiograph from outside hospital.

Patient Data

Age: 7 years
Gender: Female
x-ray

There is a well-defined lytic, very mildly expansile lesion of the distal right tibial metaphysis, extending to the level of the physis, with associated thinning of the overlying cortex. There is no associated matrix calcification or soft tissue mass.

ct

There is a large lytic lesion occupying the entire distal right tibial metaphysis, measuring approximately 2.3 x 2.5 x 4.1 cm, with associated mild expansion and marked thinning of the overlying cortex, particularly laterally and posteriorly. In the mid to upper part of the lesion, there is a small cortical breakthrough consistent with pathologic fracture.

There is no matrix calcification within the lesion, internal bony septations, or associated soft tissue mass.

Case Discussion

This is an unicameral bone cyst with a pathologic fracture in the right distal tibia as seen on CT.
