Presentation
Young female presented with h/o fall on outstretched hand and pain in the right shoulder and arm.
Patient Data
There is a sharply demarcated lytic lesion noted in the proximal humerus causing cortical expansion and thinning. The lesion is multiloculated and is associated with pathological fracture with fracture fragment can be seen in the dependent portion of cyst (Fallen fragment sign). Image findings are suggestive of simple (unicameral) bone cyst.

Typical appearance of unicameral bone cyst. The patient is typically in his 2nd decade of life, presented with multiloculated cyst in proximal humerus with pathological fracture and fallen fragment sign.
