Unicameral bone cyst - femoral neck

Diagnosis probable

Presentation

Sports injury.

Patient Data

Age: 15 years
Gender: Male

There is an eccentric relatively well-defined lytic lesion of the right femoral neck with a probable cortical fracture.

There is a multiloculated mildly expansile lytic lesion of the right femoral neck with sclerotic margin and thinning of the endosteum. A breach of the cortex is noted of the inner margin of the femoral neck (well-visualized on coronal reconstruction), indicating a pathological fracture.

The previously described lytic lesion demonstrates a low signal on T1WI, high signal on STIR with thin and regular enhancement on postcontrast sequences. No fluid-fluid levels or soft tissue component are seen. Note a periosteal reaction is noted adjacent to the pathological fracture well-visualized on sagittal and coronal sequences (STIR and postcontrast).
