Presentation
Primary infertility, lower abdominal pain
Patient Data
Retroverted, leftwards deviated unicornuate uterus. Small rudimentary uterine horn with its own endometrial cavity is seen on left side. No evidence of communication of this endometrial cavity is seen with unicornuate uterus. Small band of myometrial tissue is seen connecting the rudimentary horn with body of unicornuate uterus.
Subserosal leiomyoma, measuring approx. 10 x 9 mm is seen in posterior fundal region of unicornuate uterus. Multiple small intramural seedling myomas are seen in posterior wall and fundus.
Large elongated tortuous cystic lesion with incomplete septations, containing hemorrhage in various stages in left adnexa consistent with hematosalpinx. Left ovary is possibly seen at posterior aspect of this lesion with a small hemorrhagic cyst.
Cystic lesion seen in right ovary. Multiple follicles containing foci of hemorrhage seen adjacent to it. Elongated tortuous cystic lesion with incomplete septations seen in medial aspect of right ovary consistent with hydrosalpinx.
Minimal free fluid is seen in pelvis.
Case Discussion
Above findings are consistent with unicornuate uterus with cavitated non-communicating rudimentary horn (cavitated accessory uterine mass) and endometriosis.
