Presentation
Pelvic mass with one month of evolution, non-painful, immobile. No urinary symptoms. Normal blood count and creatinine. Urinalysis with active sediment and CRP (Protein C Reactive) in 1.8 mg / L.
Patient Data
Abdominal scan with oral and intravenous contrast medium in portal phase, which shows a cystic mass of the bladder dome at the midline, predominantly ovoid morphology, which enhances heterogeneously after administration of the IV contrast medium. It has hypoenhancing/cystic components, but these have a greater density than the urine. There are a few small curvilinear calcifications in the wall of the lesion. Loss of the fat plane between the mass and the anterior abdominal wall.
Case Discussion
The patient was taken for surgical resection with a suspected diagnosis of urachal adenocarcinoma. The pathological study of the surgical specimen reports malignant transformation of a urachal cyst.
