A large calculus axial width and coronal height up to 16 x 15 mm in distal left side ureter adjacent to UVJ with pronounced upstream hydroureteronephrosis and tortuous ureter are seen.
Right side inguinal fat hernia is also seen.
Diffuse fatty liver is noted.

The case illustrates the non-contrast MDCT features of the distal ureteric calculus with pronounced upstream hydroureteronephrosis and tortuous ureter.
