Presentation

Abdominal pain, nausea and vomiting. Increased urinary frequency and dysuria.

Patient Data

Age: 70 years
Gender: Male

There is a large, hyperdense renal calculus with coronal width up to 17x7mm in the distal segment of the left ureter adjacent the left ureterovesical junction (UVJ). Upstream hydronephrosis and hydroureter of the left collecting system. A few small calculi in the left kidney pyelocaliceal system are also seen. Note also multiple incidental dense stones within the gallbladder and a small omental fat umbilical hernia.

Case Discussion

The case illustrates a large impacted stone in distal left side ureter adjacent UVJ causing upstream obstruction (hydronephrosis and hydroureter) . Ureterolithiasis is a leading cause of colicky abdominal/flank pain and is the main cause of ureteral obstruction. Recurrent obstructive cases can lead to chronic renal disease.
