Urinary bladder clot retention

Presentation

Recent transurethral resection of prostate. Macroscopic hematuria with abdominal pain. Nurses unable to clear the urinary catheter.

Patient Data

Age: 65 years
Gender: Male

Large rounded, hyperdense mass in the distended bladder surrounding the balloon of a Foley catheter.  Mass has ROI of 68 HU and a slightly "whorled" appearance indicative of blood clot. Note gas trapped within the clot following attempts at unblocking the catheter.

Case Discussion

Appearances are typical of clot retention i.e. clotted blood with the bladder lumen encasing the Foley catheter.  This will only be resolved by bladder washout and 3-way irrigation.
