Presentation

Hematuria.

Patient Data

Age: 60 years
Gender: Female

Enhancing, lobulated soft tissue filling the left renal pelvis to the pelvoureteral junction. Symmetric enhancement of the remainder of the left ureter. Small amount of air in the bladder with mild wall thickening and perivesicular stranding. 

Case Discussion

Urothelial carcinoma of the left renal pelvis was confirmed with ureteroscopy and biopsy. For this case, it is worthwhile to consider: 

  • Do you think you would have noticed the left pelvic mass without the excretory phase imaging or leading history? It is quite subtle, but catching an incidental finding like this on a routine study performed for a different reason could save a patients life
  • As a corollary, notice how much easier the excretory phase images make identification of the mass.
