Uterine fibroids

Age: 55 years
Gender: Female

The uterus is diffusely enlarged measuring 17 x 9 x 17 cm in its maximal cephalocaudal, AP and transverse dimensions, respectively. It harbors multiple (around 15) variable-sized fibroids. They are all within the uterine fundus and body, the cervico-isthmic segment is spared. They occupy all the uterine circumference, although the largest one mostly intramural with a submucosal component, right lateral to the endometrial cavity, measuring 8.8 cm. Most of the fibroids are intramural, a few a subserosal and one is fundal pedunculated, measuring 7.3 cm in diameter. The endometrial cavity is slightly compressed by the largest interstitial myoma from its right aspect. They all show quite homogeneous T2 hypointensity, with no gross cystic degeneration. In addition, there is a cyst in left ovary.

Case Discussion

The uterus showed multiple low T2 signal lesions with mild post contrast enhancement. These lesions are compatible with uterine leiomyomas (fibroids). There are submucosal, intramural, and subserosal fibroids.
