Presentation

Acute onset pelvic pain.

Patient Data

Age: 50 years
Gender: Female

Pelvic US

ultrasound

Uterus: 11.6 x 7.3 cm. Normal echogenicity. Right uterine fibroid measuring 2.9 x 3.5 x 3.5 cm.

Endometrium: 0.4 cm.

Cervix: complex cystic and solid-appearing heterogeneous lesion at the cervix measuring 2.8 x 2.3 x 2.6 cm.

Left ovary: 9.0 ml, within normal limits.

Right ovary: 16.4 ml, dominant follicle measuring 2.0 x 2.0 x 2.2 cm.

Adnexa: no adnexal masses are appreciated.

Free fluid: none.

IMPRESSION:

  1. complex cystic and solid heterogeneous lesion at the cervix/endocervical canal

  2. fibroid uterus

Case Discussion

This represents a case of uterine leiomyoma. The patient is a 50-year-old female with no significant medical history who presented to the emergency department with severe pelvic pain. A pregnancy test performed was negative, and a pelvic ultrasound revealed a fibroid uterus. She was treated and discharged with NSAIDs. Surgery was recommended as a last resort if her fibroid exceeded 6cm.
