Uterine leiomyoma with cystic degeneration

Presentation

Pelvic pain.

Patient Data

Age: 40 years
Gender: Female

Large well-defined intramural uterine mass of posterior corporeal location displacing the endometrial cavity anteriorly. It displays an isosignal to the myometrium on T1, mixed signal on T2 composed of central isointense areas with surrounding areas of high signal (cystic degeneration). The postcontrast sequences demonstrate a heterogeneous enhancement of the solid component and non-enhanced cystic spaces. 

Normal appearance of both ovaries. Minimal peritoneal effusion in Douglas pouch.

No pelvic lymphadenopathy.

Intramural uterine leiomyoma is the most common type of uterine leiomyoma in terms of location. 

Cystic degeneration of uterine leiomyomas is an uncommon type of degeneration and represents ~4% of all types of uterine leiomyoma degeneration.
