Uterine leiomyomas and bilateral ovarian fibromas

Diagnosis probable

Presentation

Primary infertility.

Patient Data

Age: 30 years
Gender: Female

The uterus is diffusely enlarged, showing multiple intra-mural leiomyomas (at least 4) of various size, the largest measures 7.5 X 4.5 cm. They demonstrate a low to intermediate signal intensity (as compared to the normal myometrium) on T1, low signal intensity on T2 with small areas of high signal, representing edema or cystic degeneration. The postcontrast sequences show a moderate heterogeneous enhancement.

Both ovaries show numerous small follicles as well as two lesions measuring (19 X 17 mm) on the right, and (27 X 23 mm) on the left of intermediate signal on T1 and low signal on T2 with no significant enhancement following IV contrast, representing most likely ovarian fibromas.

Case Discussion

A case of uterine leiomyomas with bilateral ovarian fibroids. The main differential diagnosis of the ovarian fibroids is thecoma and fibrothecoma.
