Uterine leiomyomata and ureterocoele

Presentation

Menorrhagia, lower abdominal pain

Patient Data

Age: 35 years
Gender: Female

Diffusely enlarged uterus is studded with numerous fibroids of varying sizes, largest reaching 6.5 cm in the anterior wall. Both ovaries are normal. Normal uterine endometrium and junctional zone. No cervical lesions. Collapsed urinary bladder shows prominence of right distal ureter which opens into a cystic dilatation intramurally within the vesical typical of a ureterocoele. The artifacts seen are due to respiration and bowel motility and can be reduced by giving medication that reduces bowel motility. 

Case Discussion

This case demonstrates the numerous intramural benign fibroids in utero and the incidental detection of a right sided ureterocele. It also highlights the importance of not getting carried away by the large positive finding (the fibroids). Always scan the field for other findings.
