Presentation
Menstruation started at the age of 13 years. The patient has chronic pelvic pain mainly during the menstrual cycle. Ultrasound reported lower pelvic mass with complex texture.

Uterus didelphys with obstructed hemivagina

Two endometrial canals are noted each draining into separate vaginal canals. No dilatation of the endometrial canals is noted.
The left hemi-vagina is patent and empty but the right hemi-vagina is obstructed and distended.
Possible communication between both uterine cervixes is noted.
Both kidneys are present in normal locations.
Case Discussion
Uterine didelphys associated with obstructed right hemivagina is usually associated with ipsilateral renal agenesis, but in the current case the kidney is present.
