Vacuum phenomenon in ankle joint

Presentation

Recurrent ankle twist and pain. Referred for an ultrasound of lateral ankle ligaments.

Patient Data

Age: 40 years
Gender: Female
ultrasound

There is normal thickness, fibrillary echopattern of the anterior talofibular ligament. There are echogenic line / tiny echogenic foci under the anterior talofibular ligament. These foci change position with ankle movements.

Case Discussion

The case shows a normal anterior talofibular ligament between lateral malleolus and talus. There is air in the ankle joint under the ligament which a normal finding.
