Complaining of a vaginal mass.
Patient Data
Well-defined ovoid cystic mass arising from the posterior vaginal wall. It displays a high signal on T1 (hemorrhagic or proteinaceous content) and a low signal on T2 with restricted diffusion on DWI/ADC. No enhancement was seen on postcontrast sequences.
Numerous small cystic lesions within the cervical stroma (Nabothian cysts).
Small unilocular left ovarian cyst.
An enhancing fistulous tract at the right perianal region coursing upwards between external and internal anal sphincters opened into the anal canal at 9 o’clock orientation (intersphincteric anal fistula, incidental finding in this case).
Case Discussion
MRI features of a well-defined cyst arising from the posterior vaginal wall with hemorrhagic or proteinous content and restricted diffusion, histologically proven as an epidermal inclusion cyst.
Epithelial or epidermal inclusion cysts are considered the most common vaginal cysts. They account for up to 23% of all vaginal cysts 1,2.
