Vasa previa - type 2 with bilobed placenta

Case contributed by Dr Muhammad Yousaf
Diagnosis certain

Presentation

32 weeks pregnant. An obstetrical scan was requested to rule out placenta previa as the placenta was low-lying in the previous mid-trimester scan.

Patient Data

Age: 25 years
Gender: Female

The placenta is bilobed, with anterior and posterior lobes at the mid and lower segments. The cord insertion is velamentous just above the internal OS, unsupported by the placental tissue. Doppler tracing of an aberrant vessel traversing over the internal OS shows a heart rate (152 b/min) similar to that of fetal heart rate (154 b/min) taken from the vessel inside the amniotic cavity.

Case Discussion

Above mentioned findings are consistent with Vasa previa type-2.
