Vascular malformation - rib

Diagnosis almost certain

Presentation

Cough and fever for about a week. Referral from the HMO states: "mass in left hemithorax on chest X ray" (image unavailable).

Patient Data

Age: 15 years
Gender: Female
ct

Expansile lesion in the posterior arch of the 6th left rib, measuring 7 x 4.7 x 5.1 cm with sharp borders and a "ground glass" matrix. No evidence of periosteal reaction or invasion of adjacent structures.
No axillary, mediastinal or hilar lymphadenopathy.
The lungs are well aerated. Indentation of left lower lobe by aforementioned costal lesion.
The thymus is visible.

Case Discussion

Total body PET-CT showed increased radiotracer uptake in the lesion but no additional lesion.

Core needle biopsy of the rib lesion:

Small cores of bland-looking vascular neoplasm with well-formed spaces lined by slightly plump endothelial cells.
Few entrapped bone trabeculae are present.
Findings consistent with hemangioma/arteriovenous malformation of bone.
