Vastus lateralis muscle tear

Diagnosis almost certain

Presentation

Trauma to lateral distal aspect of thigh - Few wks. Local swelling. Referred for local swelling USG.

Patient Data

Age: 45
Gender: Male
ultrasound

There is anechoic lesion in subcutaneous plane of distal - lateral aspect of thigh .

Echoes are noted within. Lesion is compressible. Well-defined thin capsule ( < 2 mm ) is noted around lesion.  No flow is noted in wall.

Perilesional fat is echogenic with increased vascularity.

A mobile echogenic nodule is noted within - probably fat lobule.

Size :  Cranio-caudal - 50 mm ; Transverse    - 55mm ; Thickness - 10  mm.

 Vastus lateralis  shows defect deep to above mentioned subcutanous collection - tear at myotendinous junction.

Full thickness defect is in lateral most fibers.

Defect size - 33 x 33 mm.

Retraction - 18 mm.

Hypervascularity is noted at both ends.
