Venous extradural hemorrhage at the vertex

Diagnosis almost certain

Presentation

Head trauma.

Patient Data

Age: 60 years
Gender: Male

The hematoma is lentiform in shape, locates at the vertex, crosses the sagittal suture, and displaces the superior sagittal sinus inferiorly. Some hypodensity spots within the hematoma are suggestive of acute extravasation of blood (swirl sign).

The left subarachnoid hemorrhage and parenchymal hemorrhagic contusion are also seen at the left frontal lobe. 

No skull fractures.

Case Discussion

CT findings are compatible with venous extradural hemorrhage probably bleeding from the superior sagittal sinus. Unlike typically extradural hemorrhages, which do not cross sutures and whose source is from a torn meningeal artery.
