Venous intravasation from retrograde urethrogram

Case contributed by Henry Knipe
Diagnosis certain

Presentation

Multitrauma patient. Difficulty passing catheter. Probable stricture but rule out urethral injury.

Patient Data

Age: 25 years
Gender: Male

No evidence of traumatic urethral injury. There is filling of Cowper's glands and venous intravasation, seen in the setting of high pressure injection.

No urethral stricture seen but poor distension of the posterior urethra (normal finding) means one cannot be excluded. No pubic symphysis diastasis and no fracture identified. Contrast from prior CT outlines the bladder.

Case Discussion

Filling of Cowper's glands and venous intravasation should not be confused for traumatic urethral injury. It is also seen during hysterosalpingograms.
