Presentation

6-month history of hip pain. Rule out SUFE/Perthes.

Patient Data

Age: 8 years
Gender: Male
x-ray

Normal pelvis and proximal femora.

Multiple calcific densities projected over the gas of the ascending colon. However, some are more lateral than expected for the ascending colon. Soft-tissue calcification should be considered.

MRI for vascular malformation 3 mth prior

Large subcutaneous vascular malformation extending over the chest and abdomen with internal areas of focal T1 hypointensity suggestive of calcification.

On the dynamic imaging (not all included) there are areas of enhancement in the venous phase and areas of non-enhancement.

Case Discussion

On first look, the MRI, with its large cystic spaces, would make many people think that this is a lymphatic malformation. However, there is enhancement (venous on the dynamic images) and also multiple phleboliths. Both of those imaging features should make a venous vascular malformation more likely. In many cases, these are mixed vascular malformations.
