Presentation
Low back pain
Patient Data
Rounded vertebral body lesions in L3, L5 and S1 vertebrae. They show high T1 and T2 signal, no loss of signal in T1 out-of -phase sequence and signal suppression on T1 fat sat sequence. No post-contrast enhancement.
L4-5 and L5-S1 diffuse posterior disc bulges, encroaching upon exit foramina bilaterally.
Case Discussion
Vertebral hemangioma is the most common benign vertebral lesion. It is often incidentally discovered and asymptomatic. It has a characteristic appearance on MRI with high signal intensity on T1 and T2, enough to make the diagnosis.
This case shows also the appearance of vertebral hemangioma in other sequences:
T1 fat sat: complete suppression of signal reflecting high macroscopic fatty nature
in-phase and out-of-phase sequences: No loss of signal in out-of-phase sequence
