Vertebrobasilar dolichoectasia

Presentation

Patient presented with vertigo, dysarthria and ataxic gait. Symptoms were precipitated by patient extending his neck.

Patient Data

Age: 80 years
Gender: Male

The left vertebral artery crosses the midline to the right side of the medulla then hairpin turns to join with the right vertebral artery back on the left side. The vertebral arteries are elongated and tortuous, the basilar artery is elongated and dilated (measuring 5.2mm diameter on axial view).

Case Discussion

This patient was not felt to be a candidate for surgical decompression. Despite the overall poor prognosis of vertbrobasilar dolichoectasia, dilation in this case was mild and he had remained stable for 20 years post diagnosis at time of writing.
