Presentation

Paroxysmal vertigo in a hypertensive patient.

Patient Data

Age: 60 years
Gender: Male

Dilatation with elongation and tortuosity of the left vertebral and basilar arteries (diameter = 5 mm), compressing the brainstem and left cerebellar peduncle.

Chronic small vessel ischemia (Fazekas grade 1) with moderate ventriculomegaly.

Area of left parietal gliosis with enlarged post-central sulcus (probably old infarct).

Case Discussion

MRI features of vertebrobasilar dolichoectasia, compressing the brainstem and left cerebellar peduncle.

Ischemia, cranial nerve compression, vertigo (as in this case), sialorrhea, dysphagia, tinnitus, imbalance, and others symptoms may be seen in the brainstem compression by vascular structures such as vertebrobasilar dolichoectasia 1.
