Virchow-Robin spaces (type III)

Presentation

Headache, right upper limb paraplegia for one week.

Patient Data

Age: 20 years
Gender: Male

Serpiginous tubular CSF intensity lesion in the right side of the brainstem and right middle cerebellar peduncle (3 x 3 x 1.7 cm). No diffusion restriction. No post-contrast enhancement. No surrounding edema. 
No significant mass effect over adjacent parenchyma.

Case Discussion

The MRI features are suggestive of Virchow-Robin spaces (Type III). The less likely deferential diagnosis can be a neuroepithelial cyst.
