Vogt-Koyanagi-Harada disease

Diagnosis probable

Presentation

Bilateral impaired vision and headaches. No history of trauma.

Patient Data

Age: 60
Gender: Female
mri

Bilateral thickening and enhancement of the inner wall of the eyes, corresponding to panuveitis. No scleral involvement. Multifocal bulges with relative T1 hyperintensity (compared to the aqueous humor), with no enhancement and FLAIR hyperintensities related to exudative retinal detachments in both eyes.

In the brain, there is neither leptomeningeal nor pachymeningeal enhancement.

Some focal FLAIR hyperintensities in the white matter bilaterally.

Case Discussion

In this case, the diagnosis of Vogt-Koyanagi-Harada syndrome was considered the likeliest, after excluding other differential diagnoses by both the radiologist and the ophthalmologist.

The patient recovered from her condition after local and systemic corticosteroid therapy.
