Injured fifth finger, unable to flex.
Patient Data
Dorsal dislocation of the 5th (little) finger proximal interphalangeal joint. Small bone fragment at the palmar aspect of the fifth metacarpal head.
The dislocation was reduced in the emergency department. No post-reduction x-rays were taken. The patient present 1 week later from their GP for an ultrasound with an inability to flex at the fifth finger proximal interphalangeal joint.
Volar plate avulsion fracture with a bone fragment at the palmar aspect of the proximal interphalangeal joint results in mechanical block on attempted flexion.
Bone fragment compatible with a volar plate avulsion fracture sits at the palmar aspect of the proximal interphalangeal joint. Normal alignment.
Case Discussion
This is a case of a volar plate avulsion fracture from a dorsal proximal interphalangeal joint dislocation resulting in a mechanical block to finger flexion at the proximal interphalangeal joint.
