Volar plate avulsion injury

A small avulsion fracture without frank displacement is noted in the volar aspect of the proximal middle phalanx.
Mild adjacent soft tissue swelling is also present.
Case Discussion
This is a case of volar plate avulsion injury, an avulsion injury of the proximal interphalangeal (PIP) joint that is vulnerable to hyperextension injury in the form of either a ligament tear or an intra-articular fracture.
Usually, a small fracture fragment (<40% articular surface) and/or reducible fracture with <30 degrees of flexion is generally managed conservatively with finger splinting 1.
