von Meyenburg complexes

Diagnosis almost certain

Presentation

Diffuse abdominal pain. Abnormal liver on ultrasound.

Patient Data

Age: 60 years
Gender: Female
mri

The MRI sequences demonstrate:

  • innumerable small non-communicating cystic lesions of the liver less than 7 mm, scattered in both hepatic lobes of high signal on T2 with no enhancement on postcontrast sequences
  • small simple cyst of the pancreatic head (8 mm) not communicating with the pancreatic ducts (incidental finding)
  • left renal cyst (6 x 3.8 x 3.4 cm)
  • normal gallbladder, intra-and extrahepatic biliary ducts

Case Discussion

MRI features are most consistent with von Meyenburg complexes also known as Multiple biliary hamartomas (incidental findings in this case).
