Presentation

Acute left diplopia with contralateral hemiplegia.

Patient Data

Age: 85 years
Gender: Female
mri

Left-sided midbrain area of high signal on T2 and FLAIR with restricted diffusion in keeping with an acute stroke.

Small vessel ischemic changes are noted.

Case Discussion

The clinical presentation and the MRI features are most consistent with Weber syndrome.

Weber syndrome is defined as a midbrain stroke that involves the cerebral peduncle and the ipsilateral fascicles of the oculomotor nerve with as a result an ipsilateral CN III palsy and contralateral hemiplegia or hemiparesis.
