Wernicke encephalopathy

Diagnosis almost certain

Presentation

Sudden onset of ataxia and visual disturbances in a pregnant woman (20 weeks) with known history of hyperemesis gravidarum and parenteral nutrition for acute pancreatitis during the last month.

Patient Data

Age: 20 years
Gender: Female
mri

Symmetrically increased FLAIR signal intensity involving the dorsomedial thalami, periaqueductal grey matter, mammillary bodies and the tectal plate.

These areas show DWI high signal without restriction on the ADC map and show no enhancement on the post-contrast study.

Case Discussion

Wernicke encephalopathy is an acute neurological condition due to thiamine (vitamin B1) deficiency. In this case, hyperemesis gravidarum and parenteral nutrition were the main risk factors.
