Mimics of pulmonary infiltrates

Presentation

Ongoing pneumonia treatment. Infection parameter has fallen down. Troublesome mucus in upper airways. Increasing respiratory difficulty and decreasing air saturation.

x-ray

Basal left-sided pulmonary infiltrates.

A chest CT scan was performed a few hours after the x-ray, there are no infiltrates and no pleural effusion. However, there are multiple areas of air-trapping, predominantly in the apical regions.

Case Discussion

The patient's x-ray was performed in a supine position, which leads to an exaggerated projection of basal structures including blood vessels mimicking pulmonary infiltrates.
