Large wide necked zenker diverticulum (pharyngeal pouch). This filled early on the frontal series and persisted on all other views.
Regurgitation and effortful clearing demonstrated. Tiny amount of contrast aspiration.
No other upper GI abnormality.
Case Discussion
This is an impressive example of a Zenker diverticulum / pharyngeal pouch.
These arise from a congenital weakness at the posterior larynx around C5/C6 level (Killian dehiscence).
This case was closed surgically.
