Presentation
Chronic pelvic pain in a patient with history of recurrent prostatitis. Had cholecystectomy 15 years ago.
Patient Data

The right kidney is absent. The left kidney is normal in size and location. Right seminal vesicle cysts are noted with a small cystic tubular structure extending up from the ipsilateral seminal vesicle into the upper pelvic region, ending abruptly medial to the external iliac artery, indicating most likely an atretic ureter with an ectopic orifice into the right seminal vesicle.
Diffuse hepatic steatosis is noted.
Case Discussion
Zinner syndrome is a rare Wolffian duct anomaly, comprising a triad of unilateral renal agenesis, ipsilateral ejaculatory duct obstruction, and seminal vesicle cysts.
