There is a well defined heterogeneously enhancing mass lesion measuring 4.7x4.7x3.9 cm noted in the apical segment of right upper lobe.
The mass has irregular margins and appears to infiltrate the surrounding lung parenchyma.
There is an intimate relationship of the mass with the superior vena cava, however the intervening fat plane is maintained between the mass and the mediastinum.
There is a large heterogeneously enhancing lymph node mass measuring 3.8x3.8 cm noted in the mediastinum anterior to the carina (lower right para tracheal level), which appears to encase and compress the supra azygos part of superior vena cava and right main pulmonary artery with significant narrowing.
The right superior intercostal veins and the azygos vein appears prominent suggestive of collateral circulation due to SVC obstruction.
No evidence of chest wall invasion and rib erosion noted.
The visualized abdomen appears normal.
