How do spinal cavernous malformations usually present?

A number of patterns of presentation are recognised: 1) discrete episodes of neurological deterioration with varying degress of recovery between episodes, 2) slow progression of neurological decline, 3) acute onset of symptoms with rapid decline, and 4) acute onset of mild symptoms with subsequent gradual decline lasting weeks to months.

MICROSCOPIC DESCRIPTION: The section shows a nidus of large and small calibre vascular channels. No elastic tissue is identified in the walls of these vessels. The lumen of one of the large vessels is filled with blood clot.

FINAL DIAGNOSIS: Features suggestive of vascular malformation; no evidence of tumour seen.
