Did you remember to ask for the previous study to compare? Do you routinely add ultrasound if there is a mammographic abnormality?
It is a sound imaging principle to always review previous images. Granted in this case the value may be limited. It is always good to add ultrasound when an abnormality is seen on mammography. In this case a solid component was found and confirmed as IDC on core biopsy.
What characteristics suggest DCIS on the mammogram image?
The differing densities and abnormal morphologies together with the linear ductal distribution are typical of DCIS.
The magnification MLO view of the left breast shows typical calcifications highly suggestive of DCIS. The calcifications have a linear distribution with differing densities and morphologies (pleomorphism). The distribution is limited to the upper outer quadrant of the breast. Note, the calcifications are projected over an area of increased density, which raises suspicion of an underlying mass.
