This young patient with normal carotids had a TIA, and was found to have a non-occlusive thrombus in the M1 segment on the left.

MRI and CT perfusion demonstrate no infarction, but mildly increased mean transit time.

FLAIR nicely demonstrates high signal due to slow flow.
