Differentials for this appearance?
1. CNS infections - especially cryptococcus, toxoplasmosis and tuberculosis may involve the basal ganglia. 2. Disseminated metastases - although atypical location within the deep white matter.
There are multiple (at least 7) enhancing lesions within the basal ganglia bilaterally. The largest lesion within the left internal capsule measures 16 mm in maximal dimension.
