Opportunistic infections are a frequent diagnosis in immunosuppressed patients. Cryptococcus spreads through the perivascular spaces to the basal ganglia, thalamus, and periventricular white matter. Characteristically they results in an increase in the size of the perivascular spaces that contain gelatinous content.
