Vertebral pneumatocysts are air filled cavities within the vertebral bodies similar to other intraosseous pneumatocysts seen elsewhere specially adjacent to the sacroiliac joints. CT is diagnostic and differential diagnosis include necrotic neoplasm, osteomyelitis, osteonecrosis and post surgical sequel.
